Privacy Policy

This Notice describes how your medical and personal information may be used and disclosed, and how you can get access to this information. Please review it carefully. The privacy of your health information is important to us.

Our Legal Duty

Federal and state laws, including the Health Insurance Portability and Accountability Act (HIPAA), require us to maintain the privacy of your protected health information (PHI). We are also required to provide this Notice of our privacy practices, our legal duties, and your rights regarding your health information. We are required to follow the practices described in this Notice while it is in effect. This Notice is effective July 16, 2026 and will remain in effect until we replace it.

We reserve the right to change our privacy practices and the terms of this Notice at any time, provided such changes are permitted by applicable law. Before we make a significant change, we will revise this Notice and make the new Notice available upon request. You may request a copy of this Notice at any time by contacting our office.

How We May Use and Disclose Your Health Information

Treatment

We use your health information within our practice to provide, coordinate, and manage your dental care. Treatment planning, clinical care, and the fabrication of restorations in our on-site dental laboratory are handled internally. We do not routinely disclose your health information to outside providers or laboratories.

Payment

We use your health information for billing and to obtain payment for the services we provide. If you ask us to file a claim with your dental or medical benefit plan, we will share only the information necessary to process that claim.

Health Care Operations

We use and disclose your health information for operations such as quality assessment, staff review, and administrative functions necessary to run the practice.

Appointment Reminders and Treatment Alternatives

We may contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you.

As Required by Law

We will use and disclose your health information when required to do so by federal, state, or local law.

We Do Not Disclose to Third Parties

Beyond the limited uses described above, we do not sell, rent, or disclose your health information to any third party for that party’s own purposes. Any disclosure outside our practice occurs only with your written authorization or as required by law, and you may revoke an authorization at any time in writing.

• Access. You have the right to inspect and obtain a copy of your health information, subject to certain limited exceptions.
• Amendment. You have the right to request that we amend health information you believe is incorrect or incomplete.

• Restrictions. You have the right to request restrictions on how we use or disclose your health information.
• Confidential communications. You have the right to request that we communicate with you about your health in a certain way or at a certain location.
• Paper copy. You have the right to obtain a paper copy of this Notice upon request.
• Breach notification. You have the right to be notified in the event of a breach of your unsecured protected health information.

Your California Privacy Rights (CCPA / CPRA)

For personal information we collect through this website that is not protected health information governed by HIPAA, California residents have rights under the California Consumer Privacy Act, as amended by the California Privacy Rights Act. These rights include the right to know what personal information we collect and how it is used, the right to request deletion, the right to correct inaccurate information, the right to opt out of the sale or sharing of personal information, and the right not to be discriminated against for exercising these rights.

The categories of personal information we may collect through this website include identifiers such as your name, email address, and phone number that you submit through our forms, and internet or network activity information collected through cookies and analytics tools. We do not sell your personal information. This website does run analytics and advertising tools, including Google Ads and the Meta (Facebook) Pixel, and the site-activity information those tools collect is shared for cross-context behavioral advertising as the CPRA defines that term. To opt out of that sharing, or to exercise any other right listed above, email info@drkiyan.dental and we will action it.
